Actions are raised:

  • during internal audits by auditors
  • by any staff member if they identify a problem within the system, or an area of improvement
  • in management review by management
  • following client feedback or customer complaints
  • if the quality control of a test procedure fails
  • if the quality system is not adhered to
  • if the quality system fails
