What does the colonoscopy detect that the sigmoidoscopy doesn’t? Basically, one wants to focus on advanced adenomas and wants to look at proximal versus distal. What they found is that overall it was 4% advanced proximal, 2% advanced distal. Right away you see something. The left colon harbors advanced adenomas and cancers more commonly than the proximal colon. We don’t know why that is but I’ve seen that in many studies. So sigmoidoscopy is clearly not going to be half a mammogram because there’s more action on the left side than there is on the right side.
