Tell Pitt You’re Vaccinated against COVID-19 for a Chance to Win

COVID-19 vaccinations are safe, effective and help protect people from getting sick or severely ill with COVID-19. They can also help protect people around you. If you do not have a vaccine yet, please understand why you should get vaccinated. 

Pitt students, faculty and staff who are vaccinated against COVID-19 are eligible to win prizes, including up to $2,500from the University.

Complete this Voluntary Disclosure Form to be entered for a chance to win prizes during weekly drawings starting on Wednesday, June 23. The sooner you are vaccinated and complete the survey, the more opportunities you have to win. 

The form is very short and will take less than a minute to complete. Drawings will be held each week through Aug. 6. Read more details about drawings, prizes and eligibility.

A pitt.edu login is required to verify your University status. Other employees or students will not have access to your data—individual responses will remain private.
