The output of this Fire Fighter Fatality Investigation Process is a report on the evaluation and if tested, the testing results.  The report consists of the evaluation describing the unit and the condition of each component.  Each component has a photograph included in the report.  The tests results are listed and summarized along with the Pass or Fail determination.  The final SCBA report is sent to the fire department or submitting entity, the manufacturer of the SCBA and the NIOSH Division of Safety Research or the NIOSH Division of Surveillance, Hazard Evaluations and Field Studies.  If the results of the evaluation and testing identify an issue with the SCBA outside of this incident, a field problem investigation or CPIP may be initiated.
