This table shows the association between smoking and lung cancer, taken from the historic study by Doll and Hill.

Men and women with and without lung cancer were categorized as smokers or non-smokers. Thus, the exposure was smoking; the disease outcome of interest was lung cancer.

Lung cancer among exposed individuals is far in excess of lung cancer among non-exposed individuals.
