The first American colonial doctors, obviously, had English or European medical training.  The colonies; however, did not attract the top quality physicians from the Old World because of the hazardous voyage over to the colonies, the pioneer conditions prevalent in colonial America, the lack of financial rewards, and the absence of social prestige attached to this position in the fledging colonies.
