Inflammation may play an important role both in the early stages of atherogenesis and in the late stages of plague rupture in trombogenesis. This inflammation is not necessarily caused by infection. I have proposed that inflammation may underline some of the links between insulin resistance variables and endothelial activation and with coronary heart disease itself. I have presented data suggesting that adipose tissue is an important source of proinflammatory cytokines and this may play a future important role in prevention of cardiovascular diseases particularly in obese and insulin resistance subjects.
