In the literature there is much attention on fat distribution and its relationship to insulin resistance. These pictures show the peritoneal fat grains directly into the portal circulation and metabolically much more active than subcutaneous adipose tissue. So, the constant exposure of the liver to free fatty acids being released in large amounts from this fat may explain the insulin resistance and hyperinsulinaemia of the centrally obese subjects.
