Let’s suppose that there is a medical condition for which everyone is at roughly equal risk worldwide and which each year in this country alone affects 5.3 million people and kills over 40,000. Now, let’s suppose there were an extremely effective, cheap, and non-toxic intervention to prevent this condition. It would be reasonable to assume that just about every one of us would want to take advantage of this prevention opportunity. But we don’t.
