It is a coordinated network of 8 care sites serving vulnerable youth ages 12-24 at significant risk for HIV and who have historically not accessed consistent care. The network consists of 3 hospitals, 3 community health centers, and 2 multi-service outreach agencies which provide comprehensive care including HIV CTS, street and phone outreach, nursing case management, mental health counseling, and medical care.
