We identified 21 discrete-core studies with rates from urban sites, 3 studies from rural sites, and 42 studies with mixed urban-rural catchment areas. Two further studies provide rates for both urban and rural categories. Significance tests can only be constructed for differences between the urban and mixed categorizations. We found that studies conducted in urban versus mixed urban-rural catchment areas generated significantly higher rate distributions.
