Example of a theoretical model used for the evaluation of the Cœur en santé St-Henri heart health project (Am J Public Health 1999;89:1819) conducted in Montreal between 1992 and 1997.

The model was developed jointly with the intervention and evaluation teams and served as the blueprint for the development of the health promotion program and the evaluation objectives, design and methods.

It shows the major target objectives and intermediary factors which were postulated as key to achieve program success and which required measurement for the evaluation.
