The Office of Research has combined the two Request for Prior Approval Forms (0133 For NSF and 0148 for PHS) into one form Request for Prior Approval Under the University Prior Approval System (UPAS) for NIH and NSF Grants and made the combined form available as a Word file.

Follow the guidelines below:

Check the printed version of the electronic form for legibility and to ascertain that all required information has been entered before submitting the printed form to the Office of Research.
The Office of Research will review and upload a signed form to InfoEd. The department can print the signed copy from InfoEd.
