Form 0146 Intro

The Office of Research is making Form 0146 (Request for Approval for Early Account Number) available as a Microsoft Word 2003 file.

Please note that in order for the Office of Research to process the Early Account Request (EAR) (Form 0146), you will also need to provide a Form 0202.

Follow the guidelines below before submitting a printed version of an electronic Form 0146 to the Office of Research:

Print the Form 0146 on three-part, three-colored (white, canary, and pink) carbonless, laser printer paper. This paper is available from various distributors. The Office of Research cannot accept photocopies of Form 0146, and must have the form printed on paper labeled carbonless because signatures which will be handwritten on the top sheet must be automatically transferred to the other two sheets. Please be sure to put the three-part carbonless paper through your printer correctly so that handwritten information will be transferred from the top printed sheet to the next two printed sheets.

Check the printed form for legibility and to ascertain that all required information has been entered.
