ADULT TRAUMA ALERT ACTIVATION CHECKLIST (click HERE for printable version)
I. Request by pre-hospital personnel in transit
A. All Inter-Facility "Trauma Transfers"
B. All Helicopters with "Trauma Mechanism"
II. Patients with the following vital signs:
A. Glasgow Coma Score <13 or
B. Systolic Blood Pressure <90 or
C. Respiratory Rate <10 or >29 or
D. Revised Trauma Score <11
E. Pediatric Trauma Score <9
III. Anatomy of injury:
A. All penetrating injuries to the head, neck, torso, or
extremities proximal to the elbow or knee
B. Flail Chest
C. Burns >20% total body surface area (TBSA)
D. Combination trauma with burn >10% TBSA
E. Inhalation Injury
F. Limb paralysis
IV. Mechanism of Injury:
A. Ejection from the vehicle
B. Explosions
C. High tension electrical injury/lightening
D. Jumping from a bridge
V. MercyTACC/Physician Request:
If MercyTACC staff feel that a trauma alert is needed,
but does not meet the above criteria, they should
consult with the medical command physician and
do as they decide.
ADULT INTERNAL ALERT CHECKLIST
VI. Mechanism of injury:
A. Death in same passenger compartment
B. Extrication time >20 minutes
C. Roll-over
D. High speed crash
1) Initial speed > 40mph
2) Velocity change >20 mph
3) Auto deformity >20"
4) Intrusion into passenger compartment >12"
E. Motorcycle crash >20mph with separation
F. All sled, bicycle, ATV, and snowmobile accidents
(w/hx of altered mental status or unstable vital signs)
G. Falls >20 feet
H. Hypothermia
I. Trauma patient <5 or >55 years old
(w/hx of altered mental status or unstable vital signs)
J. Trauma patients w/ cardiac or resp. hx
and complaint of chest pain or S.O.B.
K. Trauma patients with bleeding disorder or currently
taking coumadin or aspirin
L. Transport by helicopter (MEDICAL PATIENTS)
M. Drowning/Near Drowning
VII. Cardiac Arrest
VIII. Hemodynamic Instability
Adults:12 y/o BP <90 and Pulse > 120
IX. Airway Compromise
Patients who are intubated, or respiring > 35/min or
<10/min, or cyanotic, or pulse ox <88, or
assisted ventilation.
* The Adult Trauma Alert criteria will be used*
