PEDIATRIC TRAUMA ALERT ACTIVATION CHECKLIST

	The pediatric trauma alert criteria will be used for 
	patients under the age of 12					
I.	Request by Pre-hospital in transit or								
	A.   All Inter-Facility "Trauma Transfers"
	B.   All Helicopters with "Trauma Mechanism"
II.	Patients with the Following Vital Signs:								
	A.	Glasgow Coma Score		<13 or	
	B.	Pediatric Trauma Score		< 10	
	C.	Unstable Vital Signs	
		1) Heart Rate			>140 	
		2) Systolic Blood Pressure	<80	
		3) Respiratory Rate		< 10 or > 40	
III.	Anatomy of Injury:								
	A. 	All penetrating injuries to the head, neck, torso, or							
		extremities proximal to the elbow or knee							
	B.	Flail Chest							
	C. 	Burns:							
		1)  5% 3rd degree in a child of any age							
		2) 10% TBSA burn in any child <10							
		3) 20% TBSA in any child 10 - 14							
	D.	Combination trauma with burn > 10 % TBSA							
	E.	Inhalation Injury							
	F. 	Pelvic Fractures (known or suspected)							
	G.	Spine Fractures (known or suspected)							
	H.	Limb Paralysis							
	I.	Amputation Proximal to the wrist or ankle
IV.	Mechanism of Injury:								
	A.	Ejection from the vehicle							
	B.	Explosions							
	C.	High Tension electrical burning/lightening							
	D.	Jumping from a bridge							
	F.	Pedestrian or Cyclist struck by a vehicle
V.	MercyTACC/Physician Request:								
	If MercyTACC staff feel that a trauma alert is needed, but does								
	not meet the above criterial, they should consult with the 								
	medical command physician and do as they decide.								
PEDIATRIC INTERNAL ALERT CHECKLIST
VI.	Mechanism of Injury								
	A. Death in same passanger compartment								
	B. Extrication time > 20 minutes								
	C. Rollover								
	D. High speed crash								
	     1) Initial Speed > 40 MPH								
	     2) Velocity Change > 20 MPH								
	     3) Auto Deformity > 20 "								
	     4) Intrusion into passanger compartment > 12 "								
	E. MCA > 20 MPH with seperation								
	F. All sled, bicycle, ATV, and snowmobile crashes (with								
	G. Hypothermia 								
	H. Trauma patients with cardiac or respiratory history and 								
	     complaint of chest pain or SOB								
	I.  Trauma patients with bleeding disorder or currently taking								
	     coumadin or aspirin								
	J. Drowning or near drowning								
	K. Transport by Helicopter (Medical Patients)
VII.	Cardiac Arrest									
VIII.	Hemodynamic Instability	
	1 year or younger	Pulse >180 or <80					
	1-5 years of age	Pulse > 160					
	6-10 years of age	Pulse > 140					
	> 10 years of age	Pulse >120 and cool, clammy,  restless									
IX.	Airway Compromise								
	Patients <1 2 years of age who are intubated, respiring >35/min								
	or < 10/min, or cyanotic, or pulse ox < 88, or Ventilations are								
	being assisted with a BVM
