The institutional questionnaire will be completed by the Office of the Provost in conjunction with CSSD. The questionnaire is due January 15, 2007. FINAL VERSION, August 1, 2006.
Program Questionnaire Deadeline updated 12/13/06

The program questionnaire will be completed by Department Chairs in conjunction with Deans. The surveys will be available online shortly after November 15, 2006. All data should be entered into the web-based program questionnaires by January 19, 2007. The program questionnaires will be submitted by the Provost's Office by February 1, 2007.
Faculty Questionnaire Deadline updated 12/13/06

The faculty questionnaire must be completed by all faculty members designated as a Core or New faculty member in programs participating in the study. All faculty members who are eligible according to the NRC guidelines must complete the questionnaire. Usernames and passwords will be sent to all eligible faculty members via e-mail and U.S. Mail in December, 2006. Faculty questionnaires must be submitted by February 15, 2007.
Admitted-to-Candidacy Doctoral Student Questionnaire Updated 3/26/07
Click here to view the doctoral student questionnaire (.pdf) The doctoral student questionnaire is part of a pilot test to collect information important to understanding the quality of doctoral programs. Doctoral students who were admitted to candidacy prior to the fall of 2006 in the following fields will be asked to complete the questionnaire: Chemical Engineering, Economics, English, Neuroscience, and Physics. Usernames and passwords will be sent to all eligible students via e-mail and U.S. Mail in April 2007. Student questionnaires must be submitted by April 30, 2007.
Survey of Program Quality (Rating Questionnaire)

During the spring of 2007, the NRC will send a Rating Questionnaire to a random sample of faculty members in each field covered by the taxonomy. Respondents will be provided with information about 15-20 programs in their field and asked to rate the quality of each program on a five-point scale. Respondents will be given the following information about each program:
1. The program's URL
2. Racial/ethnic diversity of the faculty (percent non-white)
3. Gender diversity of the faculty (percent female)
4. Number of PhDs per year over the last five years
5. Percent of entering students who complete a doctoral degree in 10 years or less
6. Time to degree
7. Percent of students placed in academic positions (from NSF)
Only faculty members who complete the Faculty Questionnaire by February 15, 2007 will be eligible to participate in the Rating Questionnaire.

The emerging fields questionnaire will be completed by Department Chairs in conjunctions with Deans. This questionnaire should be completed only if a program has been designated as an Emerging Field in the NRC taxonomy. The Emerging Fields QuestionnairePrograms designated as Emerging Fields should only complete ; they should not complete the Program Questionnaire. If you are uncertain whether your program has been designated an emerging field, please contact the Dean's Office in your school or contact Michele Colvard in the Office of the Provost.
