Professional Accomplishments
of Faculty Librarians
2008-2009 Survey

As part of an effort to recognize the many accomplishments of ULS faculty librarians, the Faculty Affairs Committee requests that you share the following information with us about your professional accomplishments for the past year. This information is collected for the purpose of honoring the professional activities of faculty librarians. The information you provide will be shared with other ULS colleagues in an online report and at the Faculty Assembly/Recognition Event on Friday, May 8, 2009, at 2:00pm in Room 272 Hillman Library. Please make your submissions as soon as possible.

1) Name: Please include your name and official title for inclusion in the report.

2) Conferences Attended: Please list the conferences you have attended since June 1, 2008. Also, indicate any special contributions such as committee work, etc.. (List any conference presentations or poster sessions under #3 below.)

3) Presentations/Poster Sessions/Lectures: Please record the title and other pertinent information, and provide brief abstracts (no more than 2-3 sentences) of any presentations or poster sessions delivered or accepted since June 1, 2008.

4) Publications: Please record the title, other publication information, and brief abstracts (no more than 2-3 sentences) of any items which have been published or accepted for publication since June 1, 2008.

5) Other Activities: Please provide a description of any other professional activities since June 1, 2008, including community service, job-related accomplishments, awards or honors, etc..

6) Promotions/Tenure: If you wish, provide information about successful peer review, promotions, and/or new rank/job title since June 1, 2008.

7) Share!: Are you interested in making a 40-45 minute presentation to your ULS colleagues on any of the work you have listed here? If yes, please indicate the topic on which you would like to make a presentation and someone will contact you about scheduling.
