The Center for Primary Care Community-Based Research: Research Registry
Background
Much advancement in medicine has resulted from research involving the collection and analysis of the medical record information of patients. For this reason, CPCR has begun a Research Registry to aide in the recruitment of study participants and the advancement of knowledge and prevention of disease.
Eligibility
All residents of Allegheny County who are age 12 and older are eligible to enroll in the registry. This involves signing a consent form and completing a brief form covering information we need about you for the registry. If you agree to participate in the registry, your past, current, and future medical record information will be placed into the Research Registry. The form you will be asked to complete requests your contact information along with your past, current, and family medical histories. This will allow us to contact you if one of our researchers determines, through review of your medical records, that you are eligible for participation in a research study. Compensation
You will be compensated $5 for the time you spend learning about and enrolling in the Research Registry.
Privacy
We will attempt to preserve your medical record confidentiality by assigning a special research code number to your medical record information stored in the Research Registry, and by removing personal identifiers (for example, your name, social security number, medical record number) from information stored about you in the Research Registry. Information linking the research code number to your name and other personal identifiers will be stored in a separate secure location. Access to any identifiable information about you that is contained within the Research Registry will be limited to investigators associated with the Center for Primary Care Community-Based Research and their research staff.
How to Enroll
There are several ways to enroll in the Registry. CPCR has five recruitment sites at FM-Pittnet Practices. They are:

Lawrenceville Family Health Center
Matilda Theiss Health Center
Squirrel Hill Health Center
Shady Side Family Health Center
CPCR also recruits at many Pittsburgh-area health fairs, farmers markets, and cultural events. If you are a participant in either of the two studies (Prostate Cancer/Stroke or Cardiovascular Disease), you may enroll in the registry at your educational session.
